Connect with Professionalism
At Fikes, we rely on the professionals who haul our loads to help build and maintain our customer relationships. That's why we expect any carrier interested in Fikes Logistics to meet the same strict criteria and high standards of service that we demand of our owner operator fleet.
The following information is needed to set up your company as a carrier:
- A signed copy of our brokerage agreement -- see our Logistics Application.
- A copy of your opperating authority certificate
- A cargo insurance certificate for a minimum of $100,000 naming Fikes Truck Line, Inc. as a certificate holder. A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax and mail us a copy.
- A liability insurance certificate for a minimum of $1,000,000 naming Fikes Truck Line, Inc. as a certificate holder. A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax and mail us a copy.
- Worker's Compensation insurance (if applicable)
- A completed W-9 form
- Your D.O.T. rating (if applicable)
