That's why we expect any carrier interested in Fikes Logistics to meet the same strict criteria and high standards of service that we demand of our owner operator fleet. The following information is needed to set up your company as a carrier: - A signed copy of our brokerage agreement -- see our Logistics Application.
- A copy of your opperating authority certificate
- A cargo insurance certificate for a minimum of $100,000 naming Fikes Truck Line, Inc. as a certificate holder. A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax and mail us a copy.
- A liability insurance certificate for a minimum of $1,000,000 naming Fikes Truck Line, Inc. as a certificate holder. A BLANK COPY IS NOT SUFFICIENT. You must have your insurance company fax and mail us a copy.
- Worker's Compensation insurance (if applicable)
- A completed W-9 form
- Your D.O.T. rating (if applicable)
The items above can be faxed to Diann Byrd at (870) 777-2592. Once all of your paperwork is submitted, a member of our Logisitics Department will contact you to approve your application or to address details that need to be worked out.
